Membership, Gift Card & Appointment Terms & Conditions

These Terms & Conditions govern memberships, packages, gift cards, and services at SAS Therapeutics. By purchasing or using our services, you agree to the following policies:

1. Introduction

These Terms & Conditions outline our membership program, gift card rules, package policies, and appointment guidelines to ensure transparency and a positive experience for all patients.

2. Clinic Information

• Clinic Hours: Monday–Thursday: 9 AM–9 PM | Friday & Saturday: 9 AM–7 PM (by appointment only)

3. Patient Responsibilities & Wellness Policy

• Patients must provide accurate health information and keep contact and billing information up to date.
• Patients who are sick, have a fever, contagious illness, or active infection should not attend appointments. SAS Therapeutics reserves the right to reschedule appointments if illness is suspected for the health and safety of patients and staff.

4. Membership Terms

• Membership Tiers & Pricing:
  Signature Tier: One (1) 60-min session/month | $95
  Preferred Tier: One (1) 90-min session/month | $139
  Premier Tier: One (1) 120-min session/month | $179
• Membership Benefits:
  Includes all massage, bodywork, or energetic therapies
  Unlimited additional sessions at member pricing
  15% off retail products & supplements
  25% off initial wellness evaluation
  Complimentary birthday month session
  Rollover sessions accumulate indefinitely while membership is active
  Transferable sessions after 6 months of continuous membership
• Guest Privileges: Members may bring a guest at member pricing (limited to 2 visits per 6 months)
• Referral Rewards Program: Members may earn discounts on additional sessions after membership fees are paid
• Membership Freeze Option:
  May freeze for 1–3 consecutive months, max 2 times per year
  Must have 1 payment processed before eligibility
  Sessions do not accrue during freeze; existing sessions remain valid

5. Package & Gift Card Policies

• Gift Cards:
  No expiration (per state law)
  In-person redemption only
  Transferable to another individual
  Non-refundable
• Packages:
  Valid for 12 months from purchase date
  Transferable to another individual
  Non-refundable

6. Appointment Policies

• 24-Hour Cancellation Notice: Required to avoid penalty
• Late Cancellation / No-Show Fee: $65 fee or loss of session credit
• Late Arrivals:
  Over 15 min late = no-show or late cancel fee applies
  Up to 15-min grace period allowed for full session if schedule permits
  Otherwise, remaining time will be honored
• Right to Refuse Service: SAS Therapeutics reserves the right to refuse service for inappropriate behavior or safety concerns

7. Payment & Financial Policies

• Accepted payment methods:
  Credit & debit cards
  HSA/FSA cards
  Cash
  AfterPay
• Membership fees, packages, and gift cards are non-refundable

8. Privacy & HIPAA Compliance

• All patient health information is protected under HIPAA regulations
• Personal information will not be shared without written consent, except as required by law

9. Liability & Disclaimers

• SAS Therapeutics provides both medical services and non-medical wellness services.
• Medical services, such as naturopathic care, weight loss programs, and other licensed medical treatments, are provided by licensed healthcare professionals in accordance with Arizona law.
• Non-medical services, such as massage therapy, bodywork, and energetic therapies, are provided for relaxation, stress reduction, and general wellness purposes.
• Patients should follow all medical advice provided by licensed practitioners for diagnosis, treatment, or medical management.
• SAS Therapeutics is not liable for injuries or damages except in cases of gross negligence or where otherwise required by law.

10. Governing Law & Policy Changes

• These Terms & Conditions are governed by the laws of the State of Arizona
• Policies may be updated at any time; changes will be posted on this page
